Getting Started: Paperwork and Permissions

We have an online process, that is confidential and HIPAA-compliant and we’ll walk you through the necessary documentation.

It is the Client/Parent’s responsibility to be well informed of insurance coverage for
services, including but not limited to: eligibility, deductible, copay or coinsurance, out of pocket
stop loss, limitation on number of sessions allowed, preauthorization for services,in/out of network
status, etcetera. (Please note some insurances only allow a certain amount of SLP services
combined with PT and OT, clients are responsible for understanding their benefits and coverage.)
All Payments and copayments are due at time of service.

Intake

Cancellation/ NO SHOW Policy

 A minimum of 24 hours advance notice for session cancellation is required.

No-shows and canceled sessions NOT GIVEN 24 hour notice will be subject to a $50 fee. A time commitment is made to you and is held exclusively for you. Therefore you are responsible for the fee if cancellation is less than 24 hours. To provide 24 hours notice please call (208) 402-5735 (you may leave a message) and/or message us through the portal.

 

Clients must maintain at least an 80% rate of attendance over a three-month time period. If attendance drops below 80%, the client’s situation will be reviewed and services may be suspended. Low attendance rates will result in loss of time slot.

 

If you are running late, a phone call is appreciated. Arrival 15 minutes past scheduled time, may result in a missed session.
